Comprehensive Geriatric Assessment
Physiology of an older person is different from people. They are at risk of developing the multiple complex medical conditions including ischemic cardiac disease, heart failure, stroke, frailty, falls, cognitive deficits, incontinence, polypharmacy etc. If the factors leading on to these medical conditions are identified at an early stage, the quality of life of an older individual can be modified. During the comprehensive geriatric assessment, we spend an hour with the patient and conduct a thorough evaluation that helps us identifying, managing and preventing the complex medical condition and related complications. Following the assessment, we send a copy of the report to the general practitioner, so that recommended plan is put in place. We then like to follow up with the patient every six months or early at the request of their general practitioner.
